هنتكلم النهاردة بإذن الله عن ال
dysmenorrhoea او ال period pains
الاحصائيات بتقول ان
one in two women suffer from dysmenorrhoea
وبعضهم بتكون severe symptoms
تخليها ما تقدرش تتحرك ولا تروح شغلها او تمارس حياتها الطبيعية

احنا كصيادلة معرضين لأى سؤال فى الموضوع ده يعنى يدخل حد يسألك اعمل ايه او اخد ايه او ايه انواع
المسكنات اللى تستخدم وما إلى ذلك
وطبعا الصيدلى لازم يتعامل ويحافظ على ال privacy

علشان ما يبقاش فى اى نوع من الاحراج

المهم بقى
ايه اللى المفروض اعرفه منها ؟؟ او اسأل عنه ؟
• السن
• البريود بتاعتها منتظمة ولا لأ
• ايه طبيعة الالم اللى بييجى وامتى بالظبط من ميعاد البريود عندها
• هل بيكون فى اعراض تانية زى مثلا صداع او الم فى الظهر او
• nausea , vomiting
• دوخة او غيره
• هل بتعانى من اى امراض او بتاخد ادوية او كده

الاول كده عايزين نفرق بين كام حاجة
عندنا حاجة اسمها primary dysmenorrhoea
ودى بيتحصل فى عمر من 17 ل 25 سنة
ايه هى بقى ؟؟؟
pain in the absence of pelvic disease
اما بقى ال secondary dysmenorrhoea
بتكون نتيجة underlying disease
وبتبقى common
فى الستات اكتر من 30 سنة
rare فى الاقل من 25 سنة
طب ايه سببها دى ؟؟؟؟
ال common cause
بيكون PID
pelvic inflammatory disease
او endometriosis

بالنسبة لنوع الالم عندنا ال Primary dysmenorrhoea
بيبقى الالم شكله ايه ؟؟
cramping lower abdominal pain
ال pain ده
فى الغالب بيبدأ قبل نزول البريود وبيبدأ يخف تدريجيا مع نزولها .

ال secondary

بقى بتكون ازاى ؟؟؟
The pain of secondary or acquired dysmenorrhoea may occur during
other parts of the menstrual cycle and can be relieved or worsened by menstruation.

والالم ده بيحصل قبل البريود بحوالى اسبوع وبيزيد مع نزولها
وزى ما قلنا انها بتكون نتيجة لمرض اخر وبتكون عند الستات فوق ال 30 سنة
In pelvic infection, a vaginal discharge may be present
in addition to pain. If, from questioning, the pharmacist suspects secondary dysmenorrhoea, the patient should be referred to her doctor

يبقى من خلال سؤال الصيدلى هيقدر يحدد النوع ولو كان شاكك انه secondary
يبقى لازم تستشير الطبيب

Other symptoms
Women who experience dysmenorrhoea will often describe other associated
symptoms. These include
• nausea, vomiting,
• general GI discomfort, constipation,
• headache, backache, fatigue,
• feeling faint and dizziness.

طيب هنتكلم دلوقتى عن حاجة اسمها
premenstrual syndrome
(PMS)
ايه ده بقى ؟؟؟؟
بيبقى عبارة عن combination
من
physical , mental symptoms associated with menstrual cycle
والاعراض دى بتظهر من 2 ل 14 يوم قبل نزول البريود
وبمجرد ما تبدأ البريود بيحصل releif
للاعراض دى
ايه هى بقى ؟؟؟؟
bloated abdomen,
increase in weight,
swelling of ankles and fingers,
breast tenderness and headaches.

Women who experience PMS describe a variety of mental symptoms
that may include any or all of irritability,
tension,
depression,
difficulty in concentrating and tiredness.

فى بعض الدراسات بتقول ان ال pyridoxine
له دور فى management of PMS
بس الميكانيزم بتاعه غير معروف
The British National Formulary states that ‘prolonged
use of pyridoxine in a dose of 10 mg daily is considered safe but the
long-term use of pyridoxine in a dose of 200 mg or more daily has been associated with neuropathy.

medications for dysmenorrhoea

اول حاجة كده بنقول ان ال pain
بيكون نتيجة زيادة البروستاجلاندنز فى ال
menstrual fluids and circulating blood of women who suffer from dysmenorrhoea.
يبقى اذن العلاج هيكون ب
analgesics that inhibit the synthesis of prostaglandins
وطبعا لازم الصيدلى زى ما قلنا يسأل هى بتاخد أدوية تانية ولا لا ممكن تتعارض مع استخدام ال
NSAIDs

هنلخص كده امتى نلجأ للدكتور
Presence of abnormal vaginal discharge
Abnormal bleeding
Symptoms suggest secondary dysmenorrhoea
Severe intermenstrual pain (mittelschmerz) and bleeding
Failure of medication
Pain with a late period
Presence of fever

نكمل بقى موضوع ال NSAIDs
NSAIDs can be considered the treatment of choice for dysmenorrhoea
وزى ما قلنا لازم نسأل ونتأكد قبل ما نديها مثلا اسبيرين ان مش عندها
asthma ولا GI disorders
لقوا ان ال
ibuprofen , diclofenac , naproxen more effective than aspirin
Naproxen 250mg
tablets can be used by women aged between 15 and 50 years for primary
dysmenorrhoea only. Two tablets are taken initially then one
tablet 6–8 hours later if needed. Maximum daily dose is 750mg and maximum treatment time is 3 days.

زى النابروجيسك والنابروفين والنابروبروفين

لكن طبعا contraindicated
فى ال peptic ulcer
وتتاخد بعد الاكل علشان نقلل ال GI upset

فى كمان ال Hyoscine
smooth muscle relaxant
يشتغل ك antispasmodic
لكن طبعا ما ننساش ال anticholinergic effects بتاعته
dry mouth,
constipation
and blurred vision

كمان ال caffeine
بيزود ال analgesic effect
بتاع ال ibuprofen لو combined معاه

فى بقى Non drug treatment

High-frequency transcutaneous electrical nerve stimulation (TENS)
ايه الفكرة ؟؟؟؟؟؟
work by altering the body’s ability to receive or perceive pain signals.
High-frequency TENS has pulses of 50–120 Hz at low intensity
effective for pain relief in primary dysmenorrhoea.

واخيرا بعض الحاجات المهمة اثناء البريود

Exercise during menstruation is not harmful
it raises endorphin levels, reducing pain and promoting a feeling of well-being.

There is some evidence that a low-fat, high-carbohydrate diet reduces breast pain and tenderness.

Take the first dose as soon as your pain begins or as soon as the bleeding starts
